At what age can my child start seeing Southern Primary Care?

Can you complete my child’s Georgia school forms?

Yes. Our providers can complete Georgia Form 3300, the vision, hearing, dental, and nutrition screening certificate required when a child first enrolls in a Georgia public school, as well as Form 3231, the state’s certificate of immunization. Just bring your child’s school forms to the appointment.

Do you offer sports physicals?

Yes. Southern Primary Care offers sports physicals for student-athletes, including the physical evaluation required by the Georgia High School Association (GHSA). GHSA physicals are valid for 12 months, and exams done on or after April 1 count for the entire next school year. Bring your child’s form to the appointment.

Can my child be seen the same day when they’re sick?

Yes. Southern Primary Care offers same-day appointments for urgent, non-emergency needs like fevers, coughs, and sore throats. Call your nearest location as early in the day as possible for the best availability. For medical emergencies, call 911 or go to the nearest emergency room.
